REQUIREMENTS

Documents needed for Submission of Compliance Monitoring Report and ICIR

1. Copy of the complete Environmental Compliance Certificate (ECC). It includes the following:
   1. Cover Letter
   2. Environmental Compliance Certificate
   3. Sworn Accountability Statement
   4. Annex A / Conditions / Environmental Management
   5. Annex B / Project Assessment Planning Tool
   6. Environmental Management Plan Summary
2. Name and contact information of the current PCO/Managing Head
3. Copy of PCO Accreditation Certificate
4. Copy of Managing Head Certificate
5. Tree planting certificate
6. Geotagged Photo of the Tree Planting Site
7. Information, Education, and Communication Campaign Documentation and/or Certification
8. Social Development Program Documentation and/or Certification
9. Copy of the Geotechnical Investigation Report (GIR), Geohazard Assessment Report (GAR), and or Engineering Geological and Geohazard Assessment Report (EGGAR), conducted by a third-party accredited by the DENR-MGB (if applicable)
10. Copy of Emergency Preparedness/Emergency Response Plan, Contingency Plan, and Preventive Maintenance to address possible environmental hazards
11. Copy of Tenurial Instrument/Proof of Authority over the project area
12. Copy of Comprehensive Land Use Plan (CLUP), or Locational Clearance (whichever is applicable)
13. Site Development Plan
14. Environmental Management Plan
15. Accomplishment Report or schedule of activities for project commencement/implementation, including photo documentation (if applicable)
16. Tree cutting permit from DENR-CENRO (required if tree cutting is unavoidable during site clearing for project implementation)
17. Disposal Permit from LGU/PLGU (required if the project transports and disposes of materials extracted during site clearing and development off-site)
18. Ambient Air Quality and Noise Level Monitoring Result (if applicable)
19. Stack Emission Test Results (if applicable)
20. Effluent Wastewater Test Results (if applicable)
21. Environmental Budget Allocations and Expenses. It includes:
    1. Environmental Impact Mitigation Plan Fund
    2. Social Development Plan Fund
    3. IEC Plan Fund
    4. Enhancement Program Fund (if applicable)
    5. Self-Monitoring Fund
