    • Activity, Description of the Project Development, Site Development.
    • Generate the Description of Management Aspects of the Project specifically; Management
    • Strategies and Policies.
    • Generate the Description of Financial Aspects of the Project specifically; Project Cost,
    • Sources of Funds, and Financial Projections.
  • Drafting of Site Development Plan
  • Geotagging of Photos and Drone Flying
  • Map Overlays
  • Survey Plan with Technical Description and duly signed by a Geodetic Engineer
  • Provision of CENRO Certification on Land Classification
  • Assistance in LGU Endorsements

REQUIREMENT FROM PROPONENT
  • Filling out the Application Form
  • Payment of Application Fee
  • Provision of Approved Survey Plan with Technical Description and duly signed by a Geodetic Engineer
  • Provision of CENRO Certification on Land Classification (if available)
  • Provision of Proof of Identity of the Leasee of the Agreement Holder
    • Individual – Philippine Statistics Authority (PSA)-certified copy of Birth Certificate
    • Naturalized Filipino citizen – Philippine Statistics Authority (PSA)-certified copy of Certificate of Naturalization
    • Corporation/Association/Partnership and the like with at least sixty percent (60%) of the capital owned by Filipino citizen/s:
      • Certified true copy of the Securities and Exchange Commission (SEC) Registration Certificate
      • Certified true copy of Articles of Incorporation
      • Certified true copy of By-Laws
      • Certified true copy of the Board resolution/Secretary’s
      • Certificate of the corporate governing body (Board of Directors, Board ofTrustees, etc.) designating the authorized representative of said corporation, association, or partnership to apply/sign documents for and on behalf of the company
    • The applicant uses a name, style, or trade name, other than the true name – Certified true copy of the certification of registration of such name, style or trade name with the Department of Trade and Industry (DTI) and SEC.
    • Cooperative – Certified true copy of Certificate of Registration with the Cooperative Development Authority (CDA)
  • Provision of Proof of Capability - Bank Certification
  • LGU Endorsement (if available)
  • Barangay Certificate stating that there are no existing indigenous people in the area
  • Duly Notarized Special Power of Attorney (SPA) (if applicable)
