RESPONSIBILITIES OF THE CONSULTANT
  • Facilitate the preparation and notarization of required application forms and statements
  • Assist in the development of the Chemical Management Plan, Emergency and Contingency Plan, and other required technical documents
  • Consolidate all environmental permits and business registration documents
  • Coordinate with the proponent to ensure all supporting documents, such as Safety Data Sheets and photo documentation, are complete and accurate
  • Submit the complete application to the appropriate DENR-EMB office and coordinate with the case handler
  • Provide guidance on compliance with CCO regulations and reporting requirements

HYDROFLUOROCARBONS (HFCs)
  • Photocopy of Environmental permits:
  • Duly accomplished and notarized application form
  • Photocopy of Environmental Permits, if applicable (e.g., ECC, HW Generator Certificate, DP, and PTO)
  • Photocopy of SEC, DTI, or CDA registration, whichever is applicable, together with the list of officers
  • Safety Data Sheet (SDS) from the manufacturing firm (for registration of a new chemical)
  • Documents describing the applicant’s handling procedure, safety precautions, and emergency response for the chemical
POLYCHLORINATED BIPHENYLS (PCBs)
  • Process/production flow chart for manufacturing operation
  • Chemical Management Plan
  • Photo documentation of the plant’s operation, storage facilities, etc.
  • Environmental Compliance Certificate (ECC)
  • Permit to Operate APSE (POA) and/or Wastewater Discharge Permit (WDP)
  • TSD Permit
  • Certificate of liabilities of parties to compensate for damage to properties and life in case of emergencies and accidents
  • Hazardous Waste Generator's ID
ASBESTOS
  • Registration with the Bureau for any person or entity who imports and manufactures asbestos and asbestos-containing products in the Philippines
  • Importation clearance from the Bureau for any person or entity who imports asbestos and asbestos-containing products
  • Registration with the Bureau for any person or entity involved in the treatment, storage, demolition, transport, and disposal of asbestos and friable asbestos-containing wastes
  • Environmental Compliance Certificate (ECC)
  • Registration with the Bureau for existing industrial, commercial, and institutional structures with sprayed-on asbestos and friable asbestos-containing materials
MERCURY AND MERCURY COMPOUNDS
