REQUIREMENTS - MINOR AMENDMENT

Change of Project Name/ Trade or Business Name

• Securities and Exchange Commission (SEC) Registration or Department of Trade and Industry (DTI) Certificate of Business Name Registration indicating new project / business / trade name

Change of Proponent/Company Name

• Amended SEC Registration or DTI Registration showing the change of company name

Transfer/Change of Ownership

• SEC Registration of New ECC holder / DTI Registration including its Board Resolution
• Legal proof of transfer / change of ownership (i.e., Deed of Sale, Deed of Assignment, Contract of Lease)
• Notarized Accountability Statement of the New ECC holder (Grantee)

Extension of deadlines for Submission of post-ECC requirements

• Supporting documents on the request to extend the deadline for submission of post ECC requirements

Extension of ECC validity

• Amended SEC Registration or DTI Registration showing the change of company name
• Justifications may include any of the following:
• Unforeseen financial capability of the proponent to continue the project within the original five (5)-year period;
• Operational constraints including a dispute with the surface owner (mining projects) or issues with informal settlers and peace and order;
• TForce majeure events (natural or human-made calamities);
• Updated baseline characteristics of the environment of the project;
• Updated sworn statement of Accountability of the Proponent;
• Updated sworn statement of Accountability of the Proponent;Updated sworn statement of Accountability of the Preparer, if any;
• Revised implementation plan in Gantt Chart for non-mining projects or development work program duly received by MGB for mining projects
• Assistance during site inspections
• Other that may be required by DENR, EMB, MGB and the EIARC

ENVIRONMENTAL IMPACT STATEMENT (eis)

Scope of Work - NEW

RESPONSIBILITIES OF THE CONSULTANT

• Prepare the EIS document in accordance with the prescribed guidelines of DENR Administrative Order No. 2003-30 together with all the modules required by the EIS System,
• Gather primary, secondary data & information and conduct analysis of key environmental impacts. Primary data shall include but not limited to the following:
• Pedology
• Groundwater Quality
• Air Quality and Noise Level
• Socio-Economic Data Health Profile
• Other studies that the EMB Review Committee may prescribed based on the findings from the EIA Technical Scoping (Subject for Additional Charges)
• Accomplish the Scoping and Procedural Screening Requirements for EIS Document
• Accomplish the environmental conditions for critical environment parameters, impact assessment & mitigation
• Facilitate and negotiate recommendation of the EIA Review Committee to the EMB-EIA Central Office. EIA Review Committee to fully sign recommendation for ECC
• Negotiate in accomplishing endorsement of the ECC to the Office of the Director. Conduct liaising works and follow ups on the processing of the ECC at EMB Central Office until its approval and release.
• Preparation of two (2) copies of the Draft EIS based on the EMB prescribed annotated outline for EIA Report for review of the proponent

• Air Pollution Control Facility Technical Drawings (Plan and Elevation with capacity)
• Plumbing Plans (Water Distribution Plan and Sewer and Storm Drainage Plans)
• Artist’s Perspective of the Project Component
• Gantt Chart or Project Timetable
• Emergency Response Policies or Operations Manual
• Other Agency Permits (e.g., Building Permit, Working Permit, Height Clearance Permit, NWRB Water Permit) whichever is applicable
• Information, Education, and Communication (IEC) Programs regarding the project.
• Barangay Resolutions in favor of the project development
