RESPONSIBILITIES OF THE CONSULTANT
- Creation of Company Registration System (CRS) and Online Permitting and Monitoring System (OPMS) account if unavailable (See separate file for requirements)
- Site visitation to assess the project
- Prepare and Generate the Engineer’s Report
- Generate duly accomplished and notarized Application Form.
- Sign and Seal from a Sanitation/Chemical Engineer
- Consolidation of documents and information needed from the client
- Consultation with regards their Wastewater Treatment Facility (WTF)
- Assist on the effluent laboratory testing and provide wastewater characteristic report
REQUIREMENTS - nEW
REQUIREMENT FROM PROPONENT
- Copy of the Government ID of the Proponent
- SEC or DTI certificates
- CRS account and OPMS (If unavailable, see separate file for the requirements)
- Copy of Environmental Compliance Certificate (ECC), Permit to Operate, and Hazardous Waste Generator ID, Pollution Control Officer (PCO) Accreditation Certificate, whichever is applicable.
- Copy of the Site Development Map, Wastewater Plan Layout, and the Plan and Elevation of the Wastewater Treatment Facility (WTF)
- Copy of monthly water consumption bill for the last twelve (12) months or average volumetric abstraction or consumption (in case of deep well, surface water, water outsourcing), whichever is applicable.
- Assistance During Site Inspections or during conduct of sampling (if applicable)
- Payment of DP Application Fee
- Payment of DP Discharge / Load Based Fee
- Other that may be required by EMB Case Handler.

- Copy of previous Discharge Permit (DP) Certificate
- SGeotagged photos of Wastewater Treatment Facility (WTF)
- Proof of compliance to DP Conditions
- Proof of Self-Monitoring Report (SMR) Submission
